Which form do I need?
- Form 1 — Resident Indian / NRI (Indian address), 18+ The everyday counter form — most adult enrolments and updates.
- Form 3 — Resident/NRI child, Indian address, 5–17 yrs Child enrolment/update, Indian address on file.
- Form 5 — Resident/NRI child, Indian address, under 5 Infant/toddler enrolment, HoF-based.
- Form 2 — NRI with foreign address, 18+ NRI whose Proof of Address is outside India.
- Form 4 — NRI child, foreign address, 5–17 yrs NRI child, Proof of Address outside India.
- Form 6 — NRI child, foreign address, under 5 NRI infant/toddler, Proof of Address outside India.
- Form 7 — Resident Foreign National, 18+ Foreign passport/OCI/valid visa holder residing in India.
- Form 8 — Resident Foreign National, under 18 Foreign national child residing in India.

Name Update
